Our Services
Our clients live with qualified caregivers (who may be a family member, excluding a spouse or legal guardian) and receive support from our professional team. We are dedicated to helping clients stay in their communities so they can continue to grow and thrive.
Eligibility requires clients to be enrolled in MassHealth and need assistance with at least one of the following Activities of Daily Living (ADLs):
Walking
Eating
Toileting
Bathing
Dressing
Transferring Services
Our staff is available around the clock to help with daily living activities, including bathing, dressing, grooming, and meal preparation. We collaborate closely with residents and their families to create personalized care plans that emphasize comfort, safety, and independence.
We provide thorough medical care for our residents, which includes medication management, support with medical appointments, and monitoring vital signs. We collaborate closely with healthcare providers to ensure our residents receive the highest standard of care.
We provide a variety of rehabilitation services, such as physical therapy, occupational therapy, and speech therapy. Our aim is to assist residents in maintaining or regaining their mobility, strength, and independence.
We believe that social interaction is a vital component of overall wellness. We offer a variety of social activities, including group outings, game nights, and arts and crafts. Additionally, we have communal areas where residents can come together and connect.
We recognize that caregiving can be demanding, which is why we offer short-term respite care for family caregivers. This service provides temporary relief, ensuring that caregivers can take a break while their loved ones receive the care and support they require.
We provide hospice care for residents who are approaching the end of life. This care focuses on offering comfort and support to both residents and their families during this challenging time.
If services include use of the client’s personal funds:
The Agency will obtain an authorization from the client/responsible party, for access to personal funds when grocery or other shopping services are to be provided and when those services include assistance with bill paying or any activities, such as shopping, that involve access to or use of client funds.
Direct Care
The AFC provider must ensure the delivery of direct care to members in a qualified setting as described in 130 CMR 408.435 by a qualified AFC caregiver, as described in 130 CMR 408.434, who lives in the residence and who is selected, supervised, and paid by the AFC provider. AFC must be ordered by a PCP and delivered by a qualified AFC caregiver under the supervision of the RN & the MDT, in accordance with each member’s written plan of care. Direct Care includes 24-hour supervision, daily assistance with ADLs and IADLs as defined in 130 CMR 408.402, and other personal care as needed.
Nursing Services and Oversight
The AFC provider must provide nursing oversight by a registered nurse who meets the qualifications as described in 130 CMR 408.433(C)(2)(a), who is not related to the member, who is licensed in MA.
Care Management
Care management must be provided by a qualified AFC care manager, as
described in 130 CMR 408.433(C)(3)(a), who is not related to the member, and who is responsible for coordinating care and monitoring the needs of the member in conjunction with the registered nurse.
